- Fill in all the information on the form, saving frequently
- Submit the document using the upload form below before the submission deadline
- A representative from the College will contact you with next steps
*You must download a copy of the Prospective Nominee Form, open it using Adobe Acrobat, and save it to your computer. Changes made to the form in a web browser cannot be saved.
Please allow yourself some time to complete the form and note that you will be required to provide references. You may also need to sit an interview with representatives of the Nominations Committee as part of the qualification process.
